If unable to print, forms are available for pickup at Leavitt Group's office. Forms are also available after hours near front entrance of the office.

*Winners will receive half of the front parking lot with access to high-volume traffic and pedestrian traffic from Woodland Park. There will be ample room to set up a shade canopy and chairs near your stand if you'd like.
